Before you start your application, please ensure that you have read the FAQs, so you know what information you need to provide when completing the online application and most importantly, you know which supporting documents we are able to accept and the specific information that must be entered on those documents. 

Please take a moment to read the detailed information about the specific type of supporting documentation you plan to provide, so you can check that your documents meet the minimum requirements needed to issue an AHC, so you have everything to hand when you start your application. 

Supporting Documentation we are able to accept:

The clinical history is probably the easiest and most straightforward way for us to verify your pet’s Microchip and Rabies Vaccination status.

The clinical history must clearly state the following information in the entry made for your pet’s rabies vaccination:

A. The Microchip Was Scanned
B. The Vaccine Manufacturer
C. The Vaccine Brand Name
D. The Vaccine Batch Number


This is all essential information that must be entered on the AHC. When contacting your veterinary surgery to request your pet’s clinical record, please ask them to check that the entry for the rabies vaccination contains all the essential information listed in A,B,C & D

Due to GDPR regulations, we are unable to request your pet’s clinical record on your behalf. You must contact your vet to request and authorise that they email the clinical record to; info@ahc.online

Some veterinary clinics issue specific Microchip Reading & Rabies Vaccination Certificates.
If your vet has provided you with one of these documents, please ensure that the certificate contains the following information:

A. The owner and pet details, including the microchip number.
B. States that the microchip was scanned at the time of rabies vaccination,
C. The rabies vaccine details, including manufacturer, brand and batch number.
D. It has been correctly certified by the vet with all of the following: 

1) The name of the vet clearly written.

2) The signature of the vet.

3) The practice stamp

We can accept Vaccination Record Cards if the entry on the vaccination card for the Rabies Vaccination has been correctly certified. To be correctly certified, the entry for the rabies vaccination must have all of the following information:

A. The owner and pet details, including the microchip number.
B. States that the microchip was scanned at the time of rabies vaccination,
C. The rabies vaccine details, including manufacturer, brand and batch number.
D. It has been correctly certified by the vet with all of the following: 

1) The name of the vet clearly written.

2) The signature of the vet.

3) The practice stamp

If you have previous had an Animal Health Certificate issued by any Official Veterinarian and the rabies vaccine entered in that AHC is still in-date, then we can use this previously issued AHC to verify ownership and your pet’s Microchip and Rabies Vaccination status. 

The page number of the AHC may differ depending on the country of entry the AHC was issued:

If the AHC was issued to enter France, please provide images of pages 1,4,8 & 9

If the AHC was issued to enter Spain, please provide images of pages 1,4,7 & 9. 

If the AHC was issued to enter the Netherlands, please provide pages 1,3,8 & 9. 

If the AHC was issued to enter Ireland, please provide image of Pages 1,3,5 & 6

The previous AHC does not need to have been issued by AHC.ONLINE. We can accept an AHC issued by any Official Veterinarian or any veterinary surgery as long as the information in the AHC is still valid and correct.

A Vet Declaration Form contains all the required information for issuing an AHC. This form can only be completed and certified by a UK registered veterinary surgeon.

Once the vet has entered all the required information confirming microchip and rabies vaccination status, they must certify the document by entering their full name, the practice stamp, signature and date. You can then provide the Vet Declaration Form when making your application for an AHC.

Please note that your vet may charge a fee to complete this document, so please enquire about this first. You can generally avoid this fee by asking your vet to email the clinical record instead.

We are able to accept UK or EU Pet Passports if the document contains evidence of an in-date and valid rabies vaccination and the vaccination entry has been correctly certified with at least the following information:

A. The Vet’s Name
B. The Vet’s Signature
C. The Practice Address
D. The Practice Telephone Number


You will see the instruction for the minimum required information printed along the spine of the rabies vaccination page in the Pet Passport – If the vet is an Official Veterinarian the entry should also include their SP number.

UK Pet Passports issued after 2014:

Please provide images of pages 4, 5, 6, 7 & 8.

UK Pet Passports issued before 2015:

Please provide images of pages 1, 2, 3, 4 & 5.

EU Pet Passports:
