Here’s what we need to to process your request
Dogs, cats and ferrets can be issued with an Animal Health Certificate if they have an in-date rabies vaccination and their microchip was scanned and verified at the time the vaccination was given. All we need is proof that this was done.
Our AHC form below will guide you through the application process. You will need to provide proof of rabies vaccination during the process. There are several ways in which you can do this:
- Vet declaration
- Previously issued Animal Health Certificate
- Vaccination card
- Clinical history
- Existing Pet Passport
- Rabies vaccination certificate
The form will tell you exactly how to provide each type of proof – you only need to provide one from this list.
